HOW TO COMPLETE THE ENROLLMENT FORM

- Current members: You must select a health insurance option, an insurance carrier and a premium level (tier) for 2011. Otherwise, you (and your currently covered dependents) will be enrolled in the Standard PPO, with coverage provided through your region’s (East, Middle or West) lowest cost carrier. You will receive your Enrollment Kit by mail in early September. If you have not received your Enrollment Kit by September 8, 2010, call 1-866-741-6464.
- Benefits-eligible employees who are NOT currently enrolled in a State-sponsored health insurance option: You have the opportunity to enroll for benefits this year. You can enroll in a health insurance option and some other benefits (e.g., dental coverage) offered through your employer. If you are interested, you can request an Enrollment Kit in early September by calling 1-866-741-6464.
- Dependent eligibility: The new health reform law extends coverage to your dependent children under age 26. You can enroll them during the open enrollment period, with coverage effective January 1, 2011. To enroll your dependents for coverage, you need to include them on your enrollment form and provide verification of their eligibility (e.g., a birth certificate copy).
- All forms must be complete, accurate and in the TTU HR office no later than 4:30 PM October 15, 2010. We will not accept any forms after that date. Incomplete forms will not be accepted by State Insurance. Employees are responsible for the accuracy, completion and delivery of all necessary documents to TTU HR by the deadline.
